Transitions Program
Enhancing the Quality of Life
Memorial Home Services offers the Transitions Program to patients with a life-limiting illness and their families. Specially trained, experienced and caring volunteers offer support, friendship and respite to patients and their families while they live with their illness.
Transitions offers case management services and our volunteers provide:
- Friendship
- Help with errands/chores/food preparation
- Transportation
- Respite care
The Transitions Coordinator will be glad to talk with you, family members, and caregivers or to your health care providers about our program. This program is free of charge.
