What is Telehealth?
Telehealth monitoring is an automated device placed in patients' homes to take their vital signs every day, remind them to take medication and answer simple questions about their condition in the comfort of their home.
How does Telehealth work?
Each day the Telehealth monitor will talk patients through checking vital signs of heart rate, blood pressure, oxygen levels and weight so that patients can be monitored on a daily basis, rather than only through scheduled home visits. Additionally, the monitor can gather other important information from patients, such as, "Are you experiencing more difficulty breathing today compared to a normal day?," "Do you feel more anxious or upset today?" The questions can be catered to the individual patient, asking for information pertaining to a specific diagnosis.
In less than three minutes, health data is easily collected in the patient's home and securely transmitted to a nurse in the office to review. If the data collected indicates a need for intervention or if the patient fails to collect health status information when prompted, Memorial Home Services will take appropriate action.
Who qualifies for Telehealth?
Candidates to be considered include patients with:
- Congestive Heart Failure
- Chronic Obstructive Pulmonary Disease
- Cardiovascular Disease
- Asthma
How does Telehealth benefit patients?
With the Telehealth monitoring system, nurses can manage patient health seven days a week. Patients can have a sense of comfort and security knowing that their health is being monitored every day. Over time Telehealth decreases hospitalization and ER visits while improving clinical outcomes.
