Customer Bill of Rights and Responsibilities
Customer Bill of Rights
We believe that all customers receiving services from Memorial Home Services should be informed of their rights.
Therefore, you are entitled to:
- Be treated with dignity, courtesy and respect.
- Receive coordination and continuity of services from referring agency to home medical equipment services.
- Be informed of charges for services and equipment, including eligibility for third party reimbursement and receive an explanation of all forms you are requested to sign.
- Receive home care equipment and services regardless of race, religion, political belief, sex, social status, age or handicap.
- Receive proper identifi cation from personnel providing services.
- Have the right to make informed decisions about proposed and on-going care or service and to be involved in planning their care.
- Have all your records and all communications, written or oral, treated confi dentially.
- Access all health records pertaining to you and to challenge and have your records corrected for accuracy.
- To have your complaints and/or suggestions heard, reviewed, and, if possible, resolved.
- Right to appropriate assessment and management of pain.
- Be assured that your rights are honored by all Memorial Home Services staff.
- The right to have their communication needs met.
- Right to privacy and security.
- Be informed of your responsibilities regarding home care equipment and services.
- The right to choose whether or not to participate in research, investigational or experimental studies, or clinical trials.
Customer Responsibilites
- Customer agrees that rental equipment will be used with reasonable care, not altered or modified, and returned in good condition (normal wear expected).
- Customer agrees to promptly report to Memorial Home Services any malfunctions or defects in rental equipment so that repair or replacement can be arranged.
- Customer agrees to provide Memorial Home Services access to all rental equipment for repair/replacement, maintenance, and/or pickup of the equipment.
- Customer agrees to use the equipment for the purpose so indicated and in compliance with the physician’s orders.
- Customer agrees to keep the equipment in their possession and at the address in which it was delivered unless otherwise authorized by Memorial Home Services.
- Customer agrees to notify Memorial Home Services of any hospitalization or change in customer insurance, address, telephone number, physician or when the medical need for rental equipment no longer exists. BILLING WILL STOP FURTHER CHARGES WHEN YOU CALL FOR PICKUP OF EQUIPMENT. BILLING IS DONE IN A MINIMUM OF MONTHLY INCREMENTS.
- Customer agrees to accept all fi nancial responsibility for home medical equipment furnished by Memorial Home Services.
- Customer agrees to inform Memorial Home Services of advance directives and/or living will/power of attorney.
- Patients should ask questions about care and service and express any concerns about their ability to follow the proposed care plan/course of treatment.
CPR Protocol
- Respiratory Therapist will perform CPR if family/caregiver desires.
- Memorial Home Service Technicians will NOT perform CPR. If a situation arises in a customer’s home that requires CPR during the time a Service Tech is present, the Service Tech’s responsibility is to ask family or caregiver if they would like for them to call for emergency assistance. Upon positive reply, the Service Tech will call 911 or local emergency number.
