Advance Directives
You have the right to express your wishes regarding the level and kinds of care you might wish to receive if you become unable to make these decisions for yourself. You may choose to express your wishes through advance directives such as a Living Will, a Durable Power of Attorney for Health Care or a Mental Health Treatment Declaration. These documents can express your choices about your future care and, if you choose, name someone to decide for you if you should become unable to speak for yourself.

If you have a written advance directive, make sure your family and your physician are familiar with it. In addition, each time you are admitted to this, or any other medical center or hospital, you should bring a copy with you at the time of registration so that it can be placed in your medical record.
COPD Disease Management Program
The Memorial Home Services COPD program helps educate and assess patients with COPD.
Through a series of home visits and phone updates, certified respiratory therapists assess patients in the home setting. Patients are educated about their disease, assessments are performed, symptoms are monitored and patients are taught exercises to help strengthen the lungs—all in the comfort of their own home. Respiratory staff works closely with medical
staff to see that primary physicians are updated as patients progress through the program.
The ultimate goal of the Memorial Home Services COPD program is to enhance the patient’s quality
of life through communication and education. A good disease management program helps to reduce overall healthcare costs, reduce rehospitalizations and reduce non-scheduled physician and ER visits. Through this program, individuals learn about their disease, learn how to better deal with symptoms, and in turn, enhance their quality of life.
